QUALIFICATIONS

:  Candidate is

required

to possess a current, valid license with the Maryland Board of Professional Counselor and Therapists.  For example:  CSC-AD or CAC-AD; minimum Trainee Certification.


JOB DESCRIPTION:

The Intake/Case Manager is responsible for providing program and admission information to prospective patients, completing admission documentation and procedures for new patients.  Service delivery shall include all or many of the following: patient screening, comprehensive psychosocial assessment, service or care planning, referral and linkage to resources, progress monitoring and reassessment, crisis intervention, and supportive counseling.  Coordinate the overall interdisciplinary plan of care for program clients. Acts as a liaison between patient and healthcare personnel to ensure necessary care is provided and patient is making progress towards treatment goals.

  •  Provides intensive management for patients, developing individualized care plans and collaborating with an interdisciplinary team, including medical and clinical professionals, to provide a comprehensive approach to care.
  • Identifies appropriate providers and facilitates in an effort to improve or maintain the social, emotional, functional and physical health status of the patient.
  • Guides patients through program onboarding process.
  • Follows up with patients to ensure their understanding of plan of care.
  • Utilizes psychosocial expertise to serve each patient's unique needs while ensuring quality and continuity of care.

Job Requirements

Qualifications

  • REQUIRED: Candidate must possess a current, valid license with the Maryland Board of Professional Counselors and Therapists; minimum Trainee Certification.  
  • Associates, Bachelors, Masters degree in Psychology, Counseling or other Human Services field required.
  • Community, case management, and/or outreach experience preferred.
