Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay. Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. Job Description

The Coordinator - Medications will be responsible for helping clients receive the proper medications needed while coordinating with contracted pharmacies.

Essential Functions

  • Primary duty is to facilitate application process for long-term medication assistance through ADAP and Pharmaceutical assistance programs for Legacy patients and others on a case-by-case basis.
  • Reviews pharmaceutical company requirements and submits as needed (collect necessary documentation from patients).
  • Receives medications, maintains log, contacts all patients to pick up, and passes meds along to physician for distribution.
  • Approve medication assistance requests at the contracted pharmacy or other pharmacy when appropriate
  • Maintains the database with updated information as medications are ordered and received.
  • Client record-keeping and reporting
  • Maintains strict confidentiality
  • Collaborate and build good work relationships with ADAP staff and Walgreens pharmacy.
  • Interaction with clients Social Workers, Case Managers, Insurance Companies and Hospitals.
  • Properly refer all clients, potential clients and telephone inquiries to other HIV Service Providers when appropriate. Every effort should be made to make the individual feel that the personnel at Legacy care about their well being and phone calls to other HIV Service Providers should be made on behalf of the inquirer by Legacy whenever possible.
  • Participates in the Performance Improvement Program.
  • Special projects as needed.

Education & Training Requirements

High School Diploma/GED required

Work Experience

  • Familiarity with HIV treatment protocols and medication resources appropriate to the HIV community.
  • High school diploma or equivalent qualifying experience
  • Must posses excellent interpersonal and communication skills
  • Knowledge of MS Excel and MS Outlook
  • Knowledge of community agencies and resources helpful
  • Knowledge of, or familiarity with Spanish is preferred.
