Child & Family Services of Southwestern Michigan, Inc. (CFSSWMI) originated in 1891 as the National Children's Home Society for the District of Michigan "to secure for every child the same love and the same opportunity that we would have our own children receive should they be left orphaned and friendless."  The first child was taken into our care on September 28, 1891, followed by hundreds of orphaned children who were helped by our agency with finding them permanent homes.

Since that time, our programs have changed periodically as new community needs were identified and as we grew to become a full-service agency serving children, families and adults. Today we offer services through five comprehensive programs: Autumn House and Harbor House Adult Day Services, Helping Hands Adult Homecare Services, Safe Shelter for Domestic Violence and West Michigan Guardianship.  We serve the people living in Berrien, Cass and Van Buren counties, with some services extending to people in eight different counties throughout Southwestern Michigan.

Autumn House is an Adult Day Service Program in need of someone who is caring, compassionate, kind, and not afraid to step out of their comfort zone to make the clients happy.

Autumn House Adult Day Services, a program of Child & Family Services of Southwestern Michigan, Inc., provides a safe and inviting daytime environment where adults with special needs can engage in meaningful and therapeutic activities which help meet their social, emotional and physical needs.

Adult Day Services Program Assistant

Position Summary

The Program Assistant will provide personal care for participants in the program, may be asked to lead activities, assist the Adult Day Activity Lead with activities, and help serve meals and snacks.  Program Assistant must be able to carry out the daily routine at the program site with limited supervision and is under the supervision of the Adult Day Services Supervisor and the Director of Programs.

Duties and Responsibilities

  • Sets up room for daily program.

  • Maintains neatness and order in rooms.

  • Assist clients in arriving and leaving program site, including removal of coats, assisting with walkers and wheelchairs.

  • Organizes, prepares, and helps distribute lunch and snacks.

  • Assists with daily activities on a regular basis, including exercise, AM and PM groups.

  • Will perform one on one interactions with the clients.
  • Assists Activity Lead with special events and parties.

  • Takes clients for walks and outings.

  • Keeps Activity Lead informed on needed supplies.

  • Socializes with clients.

  • Reports unusual behavior of clients (disposition, wandering) to Activity Lead, Nurse, or supervisor

  • Assists clients with Activities of Daily Living (ADL). Examples include: toileting, showering, feeding, exercising, and activities.

  • May open and close the premises.

  • Informs Supervisor of all schedule changes within an appropriate amount of time, i.e., calls in sick as soon as possible.

  • Communicates in an appropriate manner with clients, caregivers, and volunteers.

  • Cleans up after activities, returning items to the proper location in supplies closet.

  • Daily charting of activities, behaviors, showers, and toileting schedule.

  • Daily charting of clients goals.
  • Monitoring of clients safety and whereabouts at all time.

  • Other duties as assigned.

Job Requirements

Qualifications

  • Have a high school diploma or GED. 
  • Valid driver's license or state ID.
  • Reliable transportation to work.
  • Must be responsible and work well with others.
  • Experienced in working with the elderly, handicapped, or dementia patients is preferred.
  • C.E.N.A. Preferred
  • TB Test upon hire with followup tests as required by CFSSWMI Policy.
  • Undergo a background check with no recent felony convictions and no abuse/neglect convictions.
  • Is not listed on the Office of Inspector General Exclusion list.
